What is Polypharmacy?
Polypharmacy is the practice of administering many different medicines, especially together for the treatment of a single disease. It may also be defined as the concurrent use of multiple medications by a patient to treat usually coexisting conditions and which may result in adverse drug interactions.
What is a Structure Medication Review? (SMR)
SMRs are designed to be a comprehensive and clinical review of a patient’s medicines and detailed aspects of their health and are delivered by facilitating shared decision making conversations with patients aimed at ensuring that their medication is working well for them.
As a Primary care Network, we will identify people who would benefit most from receiving a Structured Medication Review.
The following groups have been identified as being most likely to benefit from an SMR:
