The Importance of Early Diagnosis

At our Primary Care Network (PCN), we are committed to improving early cancer diagnosis, helping our patients receive timely treatment and better outcomes. Cancer is more treatable when detected early, and by focusing on early identification, we aim to reduce the impact of cancer on individuals and their families.

Our Approach to Early Cancer Diagnosis

Our PCN takes a proactive, patient-centred approach to cancer diagnosis. We work closely with patients to identify warning signs, risk factors, and symptoms that could indicate cancer. Our healthcare team is dedicated to supporting you at every stage, from screening and early diagnosis to referral for specialist treatment if necessary.

Key services include:

  • Cancer Screening Programs:
    We encourage and facilitate participation in national cancer screening programs, such as breast, bowel, and cervical screening. These programs play a vital role in detecting cancer before symptoms develop, helping us to catch cancer early and prevent it from spreading.
  • Risk Assessments:
    We work with patients to identify risk factors for cancer, such as family history, lifestyle choices, and other medical conditions. By understanding your risk, we can offer personalised advice and support for reducing the likelihood of developing cancer.
  • Symptom Awareness:
    Educating our patients about the early symptoms of cancer is a key aspect of our work. We provide information and guidance on what to look out for, such as persistent coughs, changes in bowel habits, unusual lumps, or unexplained weight loss. Being aware of these signs can lead to earlier detection and prompt action.
  • Rapid Access to Testing:
    If cancer is suspected, we offer rapid access to diagnostic tests such as blood tests, imaging, or biopsies. We ensure that patients receive timely referrals to specialists for further investigation, minimising delays in diagnosis and treatment.
  • Multidisciplinary Care:
    Our team works closely with specialists in oncology and other areas of care to ensure that any suspected or confirmed cancer is managed in a coordinated way. This multidisciplinary approach ensures that each patient receives the most appropriate care, from diagnosis to treatment.

Cygnet PCN will work with our practices and partners to:

  • Improve referral processes across GP practices, including by introducing locally agreed, standardiSed systems for identifying people with suspected cancer, referral management, and safety netting.
  • Lead and coordinate the contributions of practices and the PCN to efforts that increase the uptake of existing National Cancer Screening programmes among our local populations.
  • Improve outcomes through reflective learning and collaboration with local partnerships, ensuring we continually improve our cancer services.

The NHS Long Term Plan aims to increase the proportion of cancers diagnosed at stages 1 and 2 from half to three quarters of all cancer patients. Achieving this goal will enable more people each year to survive their cancer for at least five years after diagnosis. Primary Care has a vital role in delivering this ambition, working closely with system partners such as Cancer Alliances, secondary care, Public Health Commissioning Teams, and the voluntary sector to ensure early detection and improved outcomes.

Raising Awareness and Encouraging Action

In addition to screening and diagnostics, we actively work to raise awareness about cancer prevention and early detection within our community. Our healthcare professionals regularly participate in campaigns to educate patients about cancer symptoms, healthy living, and the importance of regular screenings
