Tackling Health Inequalities

Cygnet PCN is deeply committed to reducing health inequalities within the communities it serves, especially those in East Riding of Yorkshire. This effort includes tailored interventions for communities facing the greatest health disparities.

These include higher levels of deprivation, fuel poverty, and childhood obesity, particularly in areas like Goole, which fall into the top 10% for service demand.

Addressing Local Needs

Data from East Riding highlights significant areas of concern, such as elevated rates of obesity in children, fuel poverty, and mental health challenges, all of which contribute to a greater demand for services​.

To address these, Cygnet PCN collaborates with public health services, voluntary organizations, and community groups to offer preventative care, including tailored health checks, weight management programs, and mental health support.

National & Local Collaboration

The NHS Long Term Plan aims to reduce inequalities in early diagnosis and access to healthcare, and Cygnet PCN is aligned with these objectives. The NHS mandates that PCNs establish clear, locally agreed actions to reduce health inequalities. These actions focus on:

  • Improving referral processes, especially for cancer and cardiovascular diseases, to ensure patients receive timely care.
  • Increasing uptake of screening programs for cancer and other critical conditions​

Cygnet PCN has also designated a Health Inequalities Lead, who plays a crucial role in integrating care and ensuring that resources target the most at-risk groups, as outlined by NHS England​

Additionally, YOURhealth coaches work with Cygnet PCN to offer personalised support for residents, focusing on lifestyle changes that can help reduce long-term health risks such as heart disease, obesity, and mental health challenge
